Participation, Liability & Media Waiver

I understand that participation in Music Play, Piano Pathway, and STEAM Creators Studio involves normal physical and group activity risks. I release Song’s Keys Music Studio and its staff from liability for injuries or illnesses except in cases of gross negligence. I confirm that my child(ren) are healthy enough to participate and that I am responsible for their supervision unless otherwise stated in the program description. I also understand that all classes may be recorded and shared only with enrolled families for educational purposes, and that these recordings may not be copied, shared, or distributed outside of the program.

I understand that photos may be taken for the purpose of documenting the program, professional education, and sharing the learning environment with the broader community. I understand that these images may be used by Song’s Keys Music Studio in print or digital formats, including the studio website, social media, newsletters, and other educational or promotional materials. Images will be used thoughtfully, and no identifying information about my child will be shared. I understand that participation in photos is voluntary, and I may choose to opt out or request that specific images not be used at any time by contacting the studio.
